
Peer Outreach Support Team (POST) provides one-to-one non-clinical case management offering meaningful, individualized support for individuals managing mental health and/or co-occurring disorders.
In this program, participants are paired with a Certified Recovery Support Practitioner (CRSP), who has personal lived experience, which helps create a foundation of trust, empathy, and understanding. POST staff provide consistent, confidential, non-judgmental, compassionate support, tailored to the participant’s needs and individual wellness/recovery goals (such as crisis navigation, exploring recovery resources, building resilience, practical coping strategies, emotional support, guidance on setting achievable goals, sharing resources, discussing challenges, and/or celebrating successes).
POST utilizes recovery-oriented, person-centered, trauma responsible, and culturally responsive approaches, as well as evidence-based practices, empowering individuals to take charge of their wellness and recovery journey by providing a strong foundation for lasting change and a roadmap for sustained wellness.
POST serves as a bridge to other community resources, offering referrals and linkages to additional services as needed, promoting a continuum of care with warm handoffs.
POST is a free service, available to Ocean County residents who are 18 years of age and older.
