How Do I Get My Donation To MHANJ Matched By My Employer?

1. Find out if your employer has a Matching Gift Program:
   · Contact your Human Resources Office directly.
2. Obtain the appropriate forms from your Human Resources Office.
3. Complete the employee's section and sign the form.
4. Send the form to MHANJ. We will complete our section, confirming receipt of your donation, and will return it to your employer.
