CORE Application and Selection Process

Anyone interested in attending the Consumer Connections CORE training will be required to complete the following process:

1.  Complete and submit the Consumer Connections CORE training application

2.  Complete and submit the appropriate referral form (below)

3.  Possible participation in a phone interview

Training Application

Consumer Connections is not currently accepting applications for its CORE training.  Please check back for more information about upcoming sessions of the CORE training.

Referral Forms

1. Employer Referral Form: This form should be completed if the applicant is employed in a NJ DMHAS funded program AND the employer is supporting the applicant’s potential attendance at the the Consumer Connections CORE training.  This form is completed by the employer.

2. Self-Referral Form: This form should be completed if the applicant is NOT employed in a NJ DMHAS funded program OR does not have the support of their employer to attend this training. The self-referral form is completed by the applicant.
